To help us provide the most effective support to your client or patient, please ensure that all referrals include the following information. This allows our team to respond quickly and appropriately, tailoring support to each individual’s needs.
Information to Include

GP and healthcare contact details
Please provide the individual’s GP or main healthcare provider’s contact details. This enables us to work in partnership with medical professionals where necessary.

Consent from the individual
It is essential that the person being referred has given their permission for their details to be shared with Essex Sight. Where appropriate, consent from a parent, guardian, or legal representative must be obtained.

Relevant medical or support history
Include any information that will help us understand the person’s circumstances, such as:

Details of diagnosed eye conditions or recent changes in vision.

Existing support services they are already accessing.

Any mobility, communication, or additional needs we should be aware of.
